A comprehensive initial evaluation includes strength and mobility testing, an in-depth injury history review, movement mechanics and gait assessment if applicable, as well as hands-on and exercise-based treatment. A plan of care and personalized home exercise program is provided. Follow-up office visits will include retesting, hands-on therapy as needed, and monitoring and progressing of the exercise plan.
60 min evaluation - $275
45 min follow up - $200

In a Telehealth visit, we will meet via video, and I will assess your movement patterns, pain, or functional limitations. I will help get you on the right track to achieving your goals within the comfort of your space in California. *Note, this service is only provided for Californian residents due to PT license limitations.

After the evaluation, I offer discounted packages when you sign up for 6 or 10 sessions and pay in full up front. My goal is not to make you the same as before you were injured; rather I aim to make you better… and then some. More consistency leads to better results. Always.
4-session package: $190/ visit ($40 savings)
8-session package: $180/ visit ($160 savings)

No. California is a direct-access state, which means you can be seen for physical therapy without a physician’s prescription. If you need care beyond 12 visits or 45 days (whichever comes first), then you need to be seen by a physician, osteopath, or podiatrist to continue physical therapy.

No. I am an out-of-network provider. Please contact your insurance to check if you have out-of-network benefits. I do offer a superbill which is an invoice to reimburse out-of-network services after they are paid for up front. Please see below for further superbill information. * Note: Medicare and Medicaid do not currently offer reimbursement for out-of-network services.

Yes! You can pay with the card given to you or I can send you a SuperBill.

Panoramic Physio is not a Medicare provider.
Because of this:Medicare will not cover services provided here.
Supplemental insurance will not pay for services that Medicare does not cover.
Claims cannot be submitted to Medicare or supplemental plans for reimbursement.
You are welcome to schedule wellness or performance sessions on a self-pay basis. These visits are not eligible for reimbursement through Medicare or insurance.
If you have Medicare and choose to proceed with cash pay, it is imperative to print and sign an Advance Beneficiary Notice (ABN) before your first appointment.

A wellness visit does not include a diagnosis code due to there being no injury. Thus a Superbill cannot be created and insurance (including out of network benefits) will not pay for these visits. While financially inconvenient, this means no referral is needed after 45 days of treatment; therefore there is more flexibility in the timeline for meeting your goals.

A Superbill is an itemized receipt given to a patient after they have paid for out-of-network services. This document includes all information an insurance company may need to reimburse for out-of-network services like provider’s ID, the patient’s diagnosis and CPT codes for service(s) rendered.
