Elevated 1:1 Care with Maddie, Board-Certified Orthopedic Specialist (OCS)
60 min Initial Evaluation — $250 Comprehensive movement analysis, objective strength and mobility testing, gait assessment (if applicable), and hands-on treatment. You will leave with a clear roadmap to optimize your movement and reach your performance milestones.
45 min Follow-Up — $200 Continuous re-testing, hands-on therapy, and active progressions to build physical resilience, restore athletic mechanics, and keep you moving forward.
Packages & Plans of Care Discounted per-session rates are available with 4 and 8-session packages to guide your progression through return-to-sport testing, performance optimization, or comprehensive rehabilitation.
Superbills for out-of-network reimbursement are happily provided upon request. See the FAQ below for details.

Initial Telehealth Evaluation | 60 min — $250 A full virtual assessment of your movement, capacity, and goals from the comfort of your home or gym. Includes:
In-depth history review, movement patterns, and functional capacity assessment
Objective movement mechanics and symptom analysis
Tailored action plan, exercise progressions, and self-treatment strategies
A personalized digital home program designed to build capacity and keep you moving forward
Telehealth Follow-Up Session | 30 min — $135 Efficient, targeted check-ins to monitor progress and elevate your performance. Follow-ups include:
Re-testing movement patterns and tracking objective milestones
Exercise form review, coaching, and active plan progressions
Superbills for out-of-network reimbursement are available upon request. Please note: Due to state physical therapy licensing regulations, telehealth services are available to California residents only.

No. California is a direct-access state, which means you can be seen for physical therapy without a physician’s prescription. If you need care beyond 12 visits or 45 days (whichever comes first), then you need to be seen by a physician, osteopath, or podiatrist to continue physical therapy.

No. I am an out-of-network provider. Please contact your insurance to check if you have out-of-network benefits. I do offer a superbill which is an invoice to reimburse out-of-network services after they are paid for up front. Please see below for further superbill information. * Note: Medicare and Medicaid do not currently offer reimbursement for out-of-network services.

Panoramic Physio is not a Medicare provider.
Because of this:Medicare will not cover services provided here.
Supplemental insurance will not pay for services that Medicare does not cover.
Claims cannot be submitted to Medicare or supplemental plans for reimbursement.
You are welcome to schedule wellness or performance sessions on a self-pay basis. These visits are not eligible for reimbursement through Medicare or insurance.
If you have Medicare and choose to proceed with cash pay, it is imperative to print and sign an Advance Beneficiary Notice (ABN) before your first appointment.

A Superbill is an itemized receipt given to a patient after they have paid for out-of-network services. This document includes all information an insurance company may need to reimburse for out-of-network services like provider’s ID, the patient’s diagnosis and CPT codes for service(s) rendered.

Physical Therapist provides SuperBill with all necessary information
